# Letter of authorization

Letter of authorization (LOA)

Phone

A signed form that lets a new phone provider move your number away from your current carrier on your behalf.

## What is Letter of authorization?

When you port a phone number, the new provider can't simply take it. Your current carrier needs proof that the account holder approved the move. The letter of authorization, or LOA, is that proof. **What an LOA usually includes:** - The account holder or business name, exactly as it appears with the current carrier. - The service address on the account. - The phone numbers being moved. - The current carrier's name and your account number. - A port-out PIN or passcode, if your carrier uses one. - The name, title and signature of someone allowed to make changes to the account, and the date. **Why ports get rejected.** The old carrier compares the LOA with its records. A different business name, an old address or a missing PIN is enough to reject the request, which adds days. Copy the details from a recent bill or ask the carrier for a customer service record first. **Some providers also ask for a copy of a recent bill** to show you own the number. **Toll-free numbers** use a similar form to change the number's managing company (its RespOrg). **Keep a copy.** If anyone questions the port later, the signed LOA shows who authorized it and when. **How Chargezen handles it:** when you bring a number to Chargezen, you sign the letter of authorization in Chargezen and follow the transfer there.

## Example

A store's LOA lists "Acme Goods LLC," but the old carrier has the account under "Acme Goods." The port is rejected. The store fixes the name to match the bill, signs again and the port goes through.

## Why Letter of authorization Matters

A correct LOA is the difference between a port that finishes on schedule and one that stalls for weeks.
